Add funding designation to donation forms in the web portal

Add the ability for users to select a funding designation to the donation forms in the web portal. This would eliminate the need for different forms for each designation and streamline the donation process.

    sdmart - It would be helpful if we could set up a single donation amount that is split between two designations. We are rolling out a program where a fixed portion of each gift will be allocated towards acquisitions while the remainder is unrestricted, and there is no way to set this up online through either Altru or XTruLink.
